HOOPS COLLEGE — YOUTH ATHLETIC PARTICIPATION, ASSUMPTION OF RISK, AND RELEASE
By signing below, I confirm that I am the legal parent or guardian of the athlete named above, and I acknowledge and agree to the following on behalf of my child and myself:
1. NATURE OF ACTIVITIES. My child will participate in basketball training, strength and conditioning, camps, clinics, team practices, team games, and travel sponsored by Hoops College, LLC ("Hoops College").
2. ASSUMPTION OF RISK. I understand that basketball and athletic training carry inherent risks including bodily injury, sprains, fractures, concussion, and in rare cases more serious injury. I voluntarily assume all such risks on behalf of my child.
3. MEDICAL DISCLOSURE. I have disclosed to Hoops College all known medical conditions, allergies, and relevant health information about my child. I authorize Hoops College staff to seek emergency medical care for my child if I cannot be reached.
4. PHOTO/VIDEO RELEASE. I grant Hoops College the right to photograph or video-record my child during activities for instructional, promotional, and internal coaching use. If I wish to opt out, I will notify Hoops College in writing.
5. RELEASE. To the fullest extent permitted by law, I release and hold harmless Hoops College, its owners, staff, coaches, and volunteers from any claim, injury, or damage arising out of my child's participation, except for injury caused by gross negligence or willful misconduct.
6. VALIDITY. This waiver is valid for one (1) year from the date signed. A renewal will be requested annually.
